Location

Midlife Midwife MKE is located at 3970 N. Oakland Avenue, Suite 703, nestled in the vibrant heart of Shorewood.. 

Parking

For your convenience, parking is available in the lot behind the building. Be sure to park in a “visitor” spot. 

Arrival

When you arrive for your scheduled appointment, enter the building and take the elevator to the 7th floor. When you enter suite 703 you can wait in the waiting area, enjoy a beverage, and . 

Your Appointment

Initial and annual well visits are one hour long, and follow-up appointments are 30 minutes.

Bring any questions or concerns you wish to discuss, as well as relevant health records and current medication/supplement/herb list.

Please fill out intake forms ahead of your appointment and any follow-up surveys after your appointment.

Labs or Tests

Midlife Midwife MKE partners with Quest Diagnositcs. Labs/paps are done in the office and Quest can bill your insurance. Referral for additional testing or radiology can be sent to your healthcare organization of choice. Midlife Midwife MKE will provide you with your results and discuss them with you.

Cancellation Policy

If you must cancel or reschedule, please do so at least 24 hours before your scheduled visit. Late cancellations and no-shows will be charged for the appointment.

When you schedule your appointment, you will provide credit card information. Your card will be charged on the day of your appointment.
