Location

Midlife Midwife MKE is located at 4050 N. Oakland Avenue, Suite 219, nestled in the vibrant heart of Shorewood, in the same building as the School of Rock. 

Parking

For your convenience, parking is available on the street, in the Walgreens parking lot across the street, and in the public parking lot just behind the Walgreens lot. 

Arrival

When you arrive for your scheduled appointment, enter the front door and go up the steps to the newly renovated waiting area. Feel free to take a seat and help yourself to water, tea or coffee. There is no need to check-in. 

Your Appointment

Initial and annual well visits are one hour long, and follow-up appointments are 30 minutes.

Bring any questions or concerns you wish to discuss, as well as relevant health records and current medication/supplement/herb list.

Please fill out intake forms ahead of your appointment and any follow-up surveys after your appointment.

Labs or Tests

Midlife Midwife MKE partners with Labcorp. Labs/paps are done in the office and Labcorp can bill your insurance. Referral for additional testing or radiology can be sent to your healthcare organization of choice. Midlife Midwife MKE will provide you with your results and discuss them with you.

Cancellation Policy

If you must cancel or reschedule, please do so at least 24 hours before your scheduled visit. Late cancellations and no-shows will be charged for the appointment.

When you schedule your appointment, you will provide credit card information. Your card will be charged on the day of your appointment.

Midife Midwife MKE is not in a network with any insurance plan. However, you may be eligible for reimbursement using your out-of-network benefits. Learn more
